Certificate of Immunization
All new students (freshman, transfers, and others) attending regularly scheduled classes or receiving resident credit will be required to submit a Certificate of Immunization prior to attending classes.
Students who have previously attended a state public college/university are not exempt from this updated, revised immunization requirement.
The requires that all students provide proof of immunity to measles, mumps and rubella. Effective January 2005, new students are also required to provide proof of immunization to varicella (chicken pox), tetanus, diphtheria and, if 18 years of age or less at the time of classes, hepatitis B. The purpose of this policy is to ensure that students are protected against communicable diseases that are preventable and to reduce the likelihood of an epidemic or threatened epidemic on a Georgia public college/university campus.
Although admission decisions will be made without the 国产自拍 Certificate of Immunization form, no student will be allowed to register for classes until all immunization requirements are met. Because of the possibility that you may be required to receive a series of vaccinations, we strongly encourage you to begin making arrangements to have the 国产自拍 Certificate of Immunization form completed by a health official a few months prior to the beginning of classes. Medical records cannot replace the form.
